Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to provide and coordinate primary care for senior members across patient residences in California. This field-based role involves exams, vaccines, chronic disease management, and close collaboration with case management and physicians.

Applicants should hold an NP or PA license in CA, with geriatric care training and relevant certificates, and be prepared for substantial travel. A patient-centered, team-oriented approach is essential.

Qualifications

• Master’s degree in Nurse Practitioner or PA program with adult/geriatric care training.
• Valid California NP license or PA-C in good standing.
• Wound Care and Palliative Care certificates are a plus.
• Knowledge of CMS guidelines and Medicare Advantage.

Responsibilities

• Provide care to an assigned panel of members in their place of residence, often in senior living communities or SNFs.
• Conduct physical/wellness exams and assessments; occasional urgent visits and telephonic triage.
• Administer vaccines and interpret diagnostics; initiate goals of care discussions.
• Develop comprehensive care plans with patients, families, and staff; coordinate services.
• Collaborate with case management for timely access to needed services.
• Document visits per established practice protocols; participate in on-call duties.
• Engage with physicians and specialists to promote preventive care and continuity.
