Job summary

Signify Health, part of CVS Health, is seeking a full-time clinician (NP or PA) to perform in-home health evaluations across a defined coverage area. Travel up to 100% may be required, with occasional overnight stays, and multi-state licensure as needed.

Visits run about 45 minutes and focus on health history, physicals, and safety in the home. Active, unrestricted licenses with board certification are required, and the role offers comprehensive benefits, paid time off, and a strong support

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Able to obtain and maintain multi-state licenses in coverage area, as required.
• Able to cover all counties within state and flex to other areas of business need.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role

• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening)
