Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

You will work in a balanced schedule with no on-call, weekends, or holidays, and compensation ranges reflect base pay plus bonus based on productivity and performance.

Qualifications

• Active ANCC or AANP family/adult/geriatric primary care certification.
• Active, unrestricted licensure in practicing state by start date.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds reconciliation, vitals, and exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps.
• Identify urgent situations and intervene as needed.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
