Job summary

Optum Home & Community Care, part of Optum, is expanding its HouseCalls program. As an Advanced Practice Clinician (NP) you will perform in-home health assessments for Medicare Advantage members, delivering preventive care with education and care coordination.

The role offers flexible scheduling (8am-8pm, 24 hours/week), no on-call or weekends, a $10,000 sign-on bonus for external candidates, and a comprehensive benefits package based on eligibility.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted licensure in the state of Texas without current disciplinary actions
• Active and unrestricted driver's license; access to reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• If applicable, path to independent practice approval with licensing board within required timelines

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage members
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
