Optum Home & Community Care is expanding its HouseCalls program, delivering in-home health assessments to Medicare Advantage members. As a HouseCalls Advanced Practice Clinician (NP or PA), you will conduct comprehensive evaluations in patients' homes, identify care gaps, document diagnoses and communicate with primary care providers.

This field-based role offers flexible scheduling, no on-call, weekends or holidays, and potential language-based incentives.

Competenze

• Active certification in Family/Adult/Geriatric Primary Care for NP or equivalent.
• Current NCCPA certification and state licensure for PA role.
• Active, unrestricted licensure in practicing state.
• Driver's license and reliable transportation for home visits.

Mansioni

• Conduct in-home assessments including history, meds reconciliation, vitals, and exam.
• Document diagnoses for care management and planning.
• Communicate findings to PCPs to address gaps in care.
• Identify urgent situations and intervene when needed.
• Educate patients on disease processes and medications.
• Address social determinants of health and provide referrals.
