Job summary

Optum Home & Community Care is expanding its HouseCalls team to bring care directly to patients at home. As an Advanced Practice Clinician (NP or PA), you will conduct in-home health assessments for Medicare Advantage and other plan members, focusing on education, gap closure, and coordination with primary care providers.

This field-based role emphasizes non-prescribing activities and holistic health, including social determinants of health.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Missouri licensure or ability to obtain by start date.
• Valid driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform physical assessments.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and treatment planning.
• Educate members on disease processes, medications and adherence.
• Coordinate with primary care providers to close care gaps and ensure safety.
