Job summary

Optum Home & Community Care, part of the Optum family, is seeking HouseCalls Advanced Practice Clinicians (NP or PA) to conduct in-home health assessments for Medicare Advantage members.

Non-prescribing, field-based role focused on education, gap closure, and collaboration with PCPs. Flexible schedules between 8am-8pm across a 7-day week, with no on-call, weekends, or holidays required.

Qualifications

• Active NP or PA credential with state licensure.
• Experience with home visits and patient education.
• Proficiency with EMR systems and technology.
• Excellent communication and cultural sensitivity.

Responsibilities

• Perform in-home assessments for Medicare Advantage and plan members.
• Document diagnoses and care plans based on assessments.
• Educate members on diseases, medications, and adherence.
• Coordinate with PCPs to close gaps in care.
• Identify urgent situations and intervene when needed.
