Job summary

Matrix Medical Network seeks highly qualified Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes nationwide. Each visit spans up to one hour, covering health history, medication adherence, and risk factors to identify care gaps.

Providers order tests as needed, coordinate with physicians and care teams, and deliver education and follow-up to support patient health outcomes. A valid NP license and strong EMR skills are required.

Qualifications

Master’s Degree or commensurate experience and NP licensure.
Current NP licensure in state of practice with prescriptive authority or ability to obtain.
AANP/AACN/ANCC board certification in Adult, Gerontology, Family or related specialties.
Current BLS/ACLS or CPR certification.
1 year NP experience preferred; new grads encouraged to apply.
Valid state driver’s license and auto insurance.
Strong computer skills and familiarity with health record software.

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnoses, treatment, and referrals.
Administer and order point‑of‑care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, and caregivers.
Coordinate with PCP on education and follow-up.
