Job summary

UnitedHealth Group's Optum HouseCalls team is seeking a Nurse Practitioner to perform in-home health assessments for Medicare Advantage members in Texas. This field-based, non-prescribing role emphasizes education, gap closure, and coordination with primary care providers.

Flexible scheduling with no on-call, weekends, or holidays. Base pay plus bonus and a comprehensive benefits package supports professional growth in a patient-centered care model.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted Texas license or ability to obtain by start date.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments (sit/stand/kneel).

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Identify urgent/emergent situations and intervene when needed.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
