Job summary

Optum Home & Community Care is seeking a HouseCalls Nurse Practitioner to conduct in-home health assessments for Medicare Advantage members. This field-based role focuses on education, gap closure, and collaboration with primary care providers.

Eligible candidates enjoy a $10,000 sign-on bonus for external applicants, flexible scheduling (8am-8pm over 7 days), and no on-call, weekends, or holidays. Join a committed, interdisciplinary team delivering care at patients' homes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Current, unrestricted license in the practicing state or able to obtain by start date.
• Driver's license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments and vital signs, medication reconciliation, history, and exam.
• Identify diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps.
• Educate members on disease management and adherence.
• Address social determinants of health and provide referrals as needed.
