Job summary

Optum Home & Community Care is hiring a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members. The role focuses on education, gap closure, and collaboration with PCPs to improve outcomes, with no prescribing required.

We offer a $10,000 sign-on bonus, flexible scheduling between 8am-8pm across a 7-day week, and 20–40 hours per week. Active ANCC/AANP certification and state licensure are required, plus reliable

Qualifications

Active licensure in the practicing state (or ability to obtain by start date)
Driver's license and reliable transportation for home visits
No disciplinary actions or practice limitations currently

Responsibilities

Conduct comprehensive in-home assessments including history, meds reconciliation, vitals, and exam
Document diagnoses for care management and treatment planning
Communicate findings to members' PCPs to address gaps in care
Identify urgent situations and intervene appropriately
Educate members on disease processes, medications, and compliance
Address social determinants of health and provide referrals as needed
