Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home Medicare assessments, working in a non-prescribing, field-based role.

You will conduct comprehensive history, exams, screenings, and educate patients, coordinating with PCPs. Flexible scheduling 8am-8pm, no on-call, weekends, or holidays.

Sign-on bonus up to $30,000 or loan repayment up to $40,000, with competitive compensation and benefits.

Qualifications

• NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• PAs: Current NCCPA certification and state licensure
• Active license without disciplinary actions
• Driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and assessments

Responsibilities

• Conduct in-home assessments, including history review, vitals, exam and tests as appropriate
• Document diagnoses for care management and plan treatment
• Communicate findings to PCPs to close care gaps
• Identify urgent situations and intervene when needed
• Educate patients on disease, meds and adherence
• Address social determinants of health and provide referrals
