Optum Home & Community Care is seeking Advanced Practice Clinicians (NP or PA) to perform in-home health assessments for Medicare Advantage members. The role emphasizes flexible scheduling, no on-call, weekends, or holidays, and a focus on education and care coordination.

Compensation includes a base range with bonus potential; sign-on bonus or student loan repayment options are available. Must hold active certification and licensure, with transportation to visit patient homes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Unrestricted licensure in practicing state and no disciplinary actions.
• Valid driver's license and reliable transportation for home visits.
• Ability to work in varied home environments and adjust to conditions.
• Eligibility to practice independently where allowed; begin process if needed.

Responsabilités

• Conduct comprehensive in-home assessments, including medical history, vitals, and medication reconciliation.
• Document diagnoses for care management and plan treatment.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
