Job summary

AmeriPharma is seeking a dedicated Registered Nurse (RN) with expertise in home infusion therapy to join our team. The RN will conduct in-home visits, provide patient education, and support treatments such as TPN and IV biologics, while collaborating with the pharmacy and healthcare team to ensure quality outcomes.

Under the Director of Nursing, you will assess needs, document care, and guide patients and families through therapy plans, aiming for adherence and optimal treatment results.

Qualifications

• Graduate of an accredited nursing school with a current RN state license.
• 2+ years of experience in home infusion and infusion centers.
• 2+ experience with IV Immunoglobulins and IV Biologics; CRNI preferred.
• Experience with peripheral IV starts, infusion therapy, and phlebotomy (adult & pediatric).
• Strong interpersonal, time management, and organizational skills.
• Excellent written and verbal communication; accurate record-keeping.
• Ability to direct information to the appropriate healthcare provider.
• Proficient computer skills including word processing, internet and email.
• CPR and BLS certifications; valid driver's license and reliable transport.

Responsibilities

• Conduct in-home infusion therapy visits per the plan of care.
• Prepare nursing notes and document patient interventions in records.
• Serve as a clinical resource and collaborate with pharmacy departments.
• Maintain knowledge of drug side effects and interactions.
• Offer telephone and in-person patient education on therapy and procedures.
• Ensure patients understand therapy plans, medications, and adherence.
• Act as patient liaison among patient, family, and pharmacy for seamless care.
• Perform head-to-toe assessments and initiate preventative care as needed.
• Use data from various sources to determine patient needs and adjust plans.
• Manage expectations regarding services, outcomes, and discharge goals.
• Develop care plans with interventions, supplies, and services.
• Regularly evaluate progress with the team and revise plans.
