Job summary

Amerita is seeking a Home Infusion Nurse to provide 1:1 infusion care in patients' homes across our service area. This role emphasizes direct patient care, IV therapy, and coordination with multidisciplinary teams to achieve optimal outcomes.

Responsibilities include assessing patients, developing care plans, educating families, and documenting care while traveling regularly to patient homes. A valid RN license and CPR are required.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Valid nursing license in state(s) of practice.
• Minimum of one year of experience as a Registered Nurse.
• Infusion and home health care experience preferred.
• Extensive IV therapy experience preferred.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• Travel up to 100% to patient homes.

Responsibilities

• Assesses all patients and establishes the Plan of Care.
• Adheres to policies addressing infusion nursing and state laws.
• Adheres to standards of practice and code of ethics as defined by the ANA.
• Collaborates with pharmacy staff, sales partners, ordering provider and patient.
• Completes patient care documentation in home or AIS.
• Manages vascular access devices and draw labs via venipuncture.
• Educates patients and caregivers.
• Coordinates nursing care with home health and community agencies.
• Participates in on-call rotation as scheduled.
• Complete expense reports and mileage tracking.
