Job summary

Amerita, Inc is seeking home infusion nurses to provide 1:1 patient care in the patient’s home. The role focuses on infusion therapies, collaboration with pharmacy, and ensuring high-quality clinical service outside the hospital setting. Responsibilities include developing and monitoring care plans, educating patients, and coordinating with home health services. On-call rotation and travel within assigned coverage area are required.

Responsibilities

• Assesses all patients assigned and establishes and implements the Plan of Care
• Adheres to policies and procedures addressing infusion nursing services and state regulations
• Adheres to INS and IgNS standards of practice and CDC guidelines
• Collaborates with pharmacy staff, sales partners, ordering provider and patient to monitor therapy goals
• Completes patient care documentation in the home or AIS
• Manages vascular access devices and peripheral infusion insertion, plus draws labs
• Educates patients and caregivers
• Evaluates progress and makes physician-recommended changes to the Plan of Care
• Coordinates nursing care with home health and community agencies
• Participates in on-call rotation per branch protocol
• Completes expense reports and mileage tracking
• Completes annual competencies and mandatory training
