Amerita, Inc. seeks a Pharmacist to join our Pharmacy team in Englewood, CO. The pharmacist will evaluate drug therapies, collaborate with physicians and nurses, and ensure compliance with all regulations while supporting home infusion patients.

The role requires a BS in Pharmacy (Doctor of Pharmacy preferred) and active pharmacist licensure, with 2+ years of acute-care or 1 year in home infusion. On-site work, competitive benefits, and shift hours apply.

Qualifications

• Pharmacist licensure in applicable states is required.
• Minimum 2 years of acute-care experience or 1 year in home infusion.
• Experience with compounding infused drugs is preferred.

Responsibilities

• Evaluate and monitor drug therapy for home care patients in collaboration with physicians and nurses.
• Adhere to policies, procedures and state/federal regulations governing pharmacy practice.
• Coordinate referrals and admissions with nurses, customer service and distribution.
• Review and verify patient orders and document accordingly.
• Assist with patient care planning and monitoring drug therapy objectives.
