Amerita, Inc. seeks a Pharmacist to join our home infusion pharmacy team in Bentonville, AR. The role reports to the Pharmacy Manager and focuses on pharmaceutical care for home care patients, ensuring safe and effective drug therapy.

You will review orders, compound sterile products per USP standards, collaborate with physicians and nurses, and participate in quality assurance activities. A PharmD or BS in Pharmacy with active licensure and 2+ years of acute-care or home infusion experience is

Qualifications

• Bachelor of Science in Pharmacy or PharmD required.
• Active Pharmacist license in practicing state(s).
• Minimum 2 years acute-care or 1 year home infusion experience.
• Experience with compounding infused drugs.
• Strong ability to evaluate and monitor drug therapies for home care patients.
• Excellent organizational and communication skills.

Responsibilities

• Evaluates and monitors drug therapy for home care patients in collaboration with physicians and nurses.
• Adheres to state/federal laws and USP standards.
• Coordinate new referrals and admissions with nurses and distribution.
• Review orders and document on order forms.
• Assist in planning patient care and monitoring therapy outcomes.
• Maintain documentation in patient records and logs.
• Oversee pharmacy technicians as required by board rules.
