Option Care Health, Inc. is seeking an entry-level Clinical Pharmacist in Englewood, Colorado.

The role involves providing safe and appropriate pharmacy services in home care, coordinating with Nursing, educating patients and caregivers, and ensuring continuity of care across settings. The ideal candidate is a graduate of an accredited School of Pharmacy with an active pharmacist license and may have 1 year of relevant experience in IV or non-sterile compounding, MTM, or completed ASHP

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: (a) ≥1 year sterile IV admixture experience; (b) non-sterile compounding, immunizations, MTM; (c) ASHP residency; (d) Advanced Pharmacy Practice Experience; (e) equivalent experience as approved by VP/SVP.

Responsibilities

• Determines patient suitability for home care; collects and organizes patient information.
• Designs and monitors patient-specific pharmacy care plans for home care.
• Modifies plans based on monitoring data and lab values.
• Provides medication-use education to staff, patients, and caregivers.
• Ensures continuity of pharmaceutical care across home and other settings.
