Amerita, Inc. in Charlotte, NC, is seeking a Pharmacist to join our home infusion pharmacy team.

The Pharmacist will report to the Pharmacy Manager and work in our City, State branch, performing pharmaceutical care activities for patients within the service area. This full-time, on-site role requires a State Pharmacist license, 2+ years acute-care or 1 year home infusion experience, and the ability to work on-call evenings and weekends.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist license in state(s) of practice.
• Minimum two years in acute-care or one year in home infusion.
• Experience with compounding infused drugs is desirable.
• Excellent verbal and written communication skills.

Responsabilités

• Evaluates and monitors drug therapy for home care patients in collaboration with physicians and nurses.
• Adheres to all applicable state and federal pharmacy laws and accreditation standards.
• Coordinates referrals and admissions with nurses, CS and distribution.
• Reviews and verifies new patient orders and documents on order forms.
• Assists physicians with planning and objectives for drug therapy; monitors safety and efficacy.
