Job summary

Amerita, Inc. seeks a Pharmacist to join our Pharmacy team in Texas. The role reports to the Pharmacy Manager and supports home infusion patients in our branch.

You will evaluate drug therapies, ensure compliance with USP standards, and collaborate with physicians, nurses, and distribution to optimize patient outcomes. A strong clinical background and flexibility for on-call duties are essential.

Qualifications

Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
Registered Pharmacist License in state(s) of practice.
Minimum of two (2) years of acute-care experience or one (1) year in home infusion.
Experience with compounding infused drugs.
Flexibility to work on-call evenings and weekends.

Responsibilities

Evaluates and monitors drug therapy for home care patients with healthcare team.
Adheres to policies, procedures and applicable laws/regulations.
Coordinate new referrals/admissions with nurses, service, and distribution.
Review and verify all new patient orders and document appropriately.
Assist physicians and healthcare professionals with patient care planning and monitoring.
