Job summary

Soleo Health is seeking a Full-Time Infusion Nurse to support infusion patients in Elgin, IL and surrounding areas. The nurse will provide direct patient care, assess and educate patients, and coordinate home infusion therapy with the care team.

The role requires nursing experience, infusion therapy background, and ability to work in home or alternate care settings. Mileage reimbursement is provided and on-call rotation is required.

Qualifications

• Active RN license in the state's Board of Nursing.
• BSN degree preferred.
• Minimum 3 years nursing experience with background in infusion therapy preferred.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy.
• Evaluate the home environment and identify interventions for safe care.
• Develop and evaluate the plan of care with the team.
• Manage schedule via company platform and inform changes.
• Coordinate patient care via approved platform.
• Participate in on-call rotation for continuous nursing support.
• Document encounters and care activities per policy and regulatory requirements.
