Job summary

Soleo Health is seeking a Full-Time Infusion Nurse to support infusion patients in Elmhurst, IL and surrounding areas. You will provide direct patient care, assessments, care planning, and education for home infusion therapy.

The role requires on-call rotation, driving to patient homes, and collaboration with pharmacy, prescribers, and care partners. A current RN license is required; BSN preferred; 3+ years infusion experience is ideal; MS Outlook/Office proficiency; valid driver's license.

Qualifications

• Current RN license from the applicable state's Board of Nursing.
• BSN degree preferred.
• Advanced certifications (e.g., CRNI, ONS) preferred but not required.
• Participation in an on-call rotation, including nights and weekends.
• Administrative oversight of workflows and resources.
• Emergency availability during non-standard hours as required.
• Minimum 3 years nursing experience; infusion experience preferred.
• ICU or Cardiac Care background preferred.
• Proficient with Microsoft Outlook and Office.
• Valid state driver's license with no significant restrictions.

Responsibilities

• Provide direct care for assigned patients, including initiation and maintenance of communications with care team members.
• Educate patient and caregiver to safely administer home infusion therapy.
• Assess home environments and ensure safety for prescribed care.
• Develop and evaluate the care plan with the team.
• Coordinate care via approved company platform and update schedules as needed.
• Participate in the nurse on-call rotation to support continuous care.
• Document encounters and care activities per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure timely submission of POTs and required clinical documents.
