Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion suite patients in Woodridge, IL. The Infusion Nurse provides direct patient care, assessments, care planning, evaluation, and education for patients receiving infusion therapy in home or alternate sites.

Responsibilities include initiating and coordinating care with the care team, educating patients and care partners, and ensuring documentation, safety, and compliance.

Qualifications

• Current RN license in good standing.
• BSN preferred from an accredited program.
• Advanced infusion certifications (CRNI/ONS/IgRN) preferred.
• Minimum 3 years nursing experience with infusion therapy preferred.
• Proficiency with Microsoft Outlook and Office; valid driver's license.

Responsibilities

• Provide direct care for assigned patients and coordinate with care team.
• Educate patients and care partners to safely administer home infusion therapy.
• Assess home environment and implement safety measures for prescribed care.
• Develop and update plan of care with the team and evaluate response.
• Document encounters and maintain compliance with policy and regulations.
