Job summary

Soleo Health is seeking a Full-Time Infusion Nurse to support infusion patients in Elmhurst, IL and surrounding communities. You will provide direct patient care, assessments, and education in home or alternate site settings, following care matrices and clinical standards.

The role requires current RN licensure, 3+ years in infusion or ICU/cardiac care preferred, and willingness to participate in on-call rotations and travel up to an hour to patient homes.

Qualifications

• Current RN license by the state Board of Nursing in good standing.
• BSN preferred from an accredited institution.
• Advanced certification in specialty areas (e.g., CRNI, ONS) preferred but not required.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communication with the care team.
• Educate patients/care partners for safe home infusion therapy with instructions as indicated.
• Assess home environment for safety and appropriateness of prescribed care.
• Develop and evaluate the care plan with the team for each encounter.
• Coordinate patient care using approved company platform and schedule changes as needed.
• Participate in the nurse on-call rotation to ensure continuous support.
