Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support infusion patients in Duluth, GA and surrounding areas. The RN administers infusion therapy, conducts assessments, and educates patients in a home or alternate site setting.

Responsibilities include coordinating care with pharmacists and prescribers, maintaining care plans, and ensuring timely documentation. On-call rotations and travel within a 1-hour radius are required.

Qualifications

• Current RN license by state Board of Nursing.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communications with care team.
• Educate patients/care partners to safely administer home infusion therapy.
• Assess home environment and ensure safety for prescribed care.
• Develop and evaluate plan of care with the team.
• Coordinate patient care via approved platform and document encounters per policy.
• Participate in on-call rotation to provide continuous nursing support.
• Maintain knowledge of Soleo therapies, protocols and standards.
