Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support infusion patients in the Lawrenceville, GA area. The role involves direct patient care, home infusion assessments, and collaboration with a multidisciplinary team.

Responsibilities include patient education, care planning, and ensuring regulatory documentation. A strong emphasis on safety, scheduling, and on-call availability is required to effectively support our patients and adapt to the home infusion setting.

Qualifications

• Active RN license in good standing.
• BSN degree preferred.
• Infusion therapy experience preferred.
• ICU or Cardiac care experience preferred.
• On-call rotation availability required.
• Strong documentation and regulatory compliance.

Responsibilities

• Provide direct care for infusion patients, initiate and maintain communications with care team.
• Educate patient/care partner on home infusion therapy and provide additional instruction as indicated.
• Assess home environment and identify interventions to ensure safe care.
• Develop and evaluate plan of care with the team and assess response to therapy.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care through approved company platform.
• Participate in nurse on-call rotation to provide continuous nursing support.
• Document all patient encounters and activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POTs and clinical documents per policy.
• Collaborate with departments to obtain required patient documents.
