Job summary

Soleo Health Inc is seeking a Full-time Infusion Nurse to support infusion patients in the Denver, CO area. The role involves direct patient care, assessments, care planning, and education for home infusion services.

You will coordinate with the care team, maintain documentation, manage schedules, and participate in on-call rotation. RN license and infusion experience are required or preferred; BSN encouraged.

Qualifications

• Current RN license in good standing.
• BSN preferred from an accredited institution.
• Advanced certifications in CRNI, ONS, or IgRN preferred.
• Minimum 3 years nursing experience; infusion therapy preferred.
• Experience in ICU and/or Cardiac Care preferred.
• Proficient with Microsoft Outlook and Microsoft Office Suite.
• Valid driver’s license with no significant restrictions.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with care team members.
• Educate patient/care partner to safely administer home infusion therapy and provide additional instruction.
• Evaluate home environment and identify safety interventions for prescribed care.
• Develop and evaluate plan of care with other team members; assess response to therapy.
• Manage schedule via company platform and communicate scheduling changes.
• Coordinate patient care via approved company platform.
• Participate in nurse on-call rotation to ensure continuous support.
• Document all patient encounters and care activities per policy.
• Maintain knowledge of Soleo therapies, protocols, and standards.
• Ensure timely submission of POTs and required documents per policy.
• Work with departments to complete patient documents.
• Perform other duties as assigned.
