Job summary

Ssoleo Health Inc. seeks a Full-time Infusion Nurse to support infusion patients in Columbine, CO and surrounding areas. The role provides primary patient care, assessments, care planning, and education for home infusion therapy in community settings.

The position requires an RN in good standing in Colorado, infusion experience, and the ability to travel to patient homes. We offer mileage reimbursement, paid drive time, and a comprehensive benefits package.

Qualifications

• Active Colorado RN license in good standing.
• BSN preferred.
• Minimum 3 years nursing with infusion background.
• ICU or Cardiac care experience preferred.
• Proficient with Office tools and care coordination software.

Responsibilities

• Provide direct patient care for infusion therapy at home or alternate sites.
• Educate patient and care partner on safely administering home infusion therapy.
• Evaluate home environment and ensure safety for prescribed care.
• Develop and evaluate care plans with the team.
• Manage scheduling and coordinate care via approved platforms.
• Participate in nurse on-call rotation for continuous support.
• Document encounters per policy and regulatory requirements.
• Maintain knowledge of Soleo therapies and protocols.
• Ensure timely submission of POTs and clinical documents.
• Collaborate with departments to complete patient documents.
