Job summary

Soleo Health is seeking a part-time Infusion Nurse to support infusion patients in Firestone, CO, and surrounding areas. You will provide direct patient care, manage care plans, educate patients, and coordinate home infusion services in a flexible schedule.

Requires RN license in Colorado, 3+ years clinical nursing experience, and infusion therapy expertise. Travel up to 2 hours with mileage reimbursement is provided along with an hourly pay rate.

Qualifications

• Current RN license in the State of Colorado in good standing.
• BSN from an accredited program preferred.
• Minimum 3 years of clinical nursing experience; infusion therapy experience preferred.

Responsibilities

• Provide direct care for assigned patients including initiation and maintenance of communication with care team members.
• Educate patient and care partner to safely administer home infusion therapy and provide instruction as needed.
• Assess home environment and implement interventions to ensure safe care delivery.
• Develop and evaluate plan of care with the team and monitor response to therapy.
• Manage scheduling and coordinate patient care via approved company platform.
• Be available for nurse on-call rotations to support continuous care.
