Job summary

Option Care Health is a leading home infusion provider seeking an Infusion Nurse II to coordinate and deliver intermediate patient care in the home or infusion suite. You will implement treatment plans, adjust as needed, and educate patients and families to promote safe, effective care under a physician's guidance.

The role requires current RN licensure, CPR certification, strong computer and documentation skills, and willingness to travel up to client locations.

Qualifications

• Active RN license in the practice state.
• CPR certification required.
• Proficiency with basic computer software and EMR systems.
• Willingness to travel to home visit locations as needed.

Responsibilities

• Coordinate and provide intermediate direct patient care in home or infusion suite.
• Implement and adjust plans of care with physician coordination.
• Document care in real-time and ensure billing-ready records.
• Educate patients and caregivers and mentor junior nurses when needed.
• Participate in multidisciplinary care planning and handoffs.
