Job summary

Qualifications

• Willingness to travel up to 100% for business purposes.

Responsibilities

• Initiates and coordinates intermediate nursing plan of care treatments in home or infusion suite.
• Educates patients and families, documents care in real time, and ensures compliance with physician plan of care.
• Provides training and mentorship to junior nurses as needed.
• Communicates with clinical team to optimize care and billing processes.
• Performs discharge planning and prepares summaries as required.
