Job summary

Naven Health, Inc. is seeking an Infusion Nurse I to provide direct patient care in patients' homes or alternate infusion sites, under supervision.

You will administer treatments, educate patients and families, and ensure safety under a physician's plan of care. Responsibilities include coordinating with the Clinical team, documenting care in real time, and supporting discharge planning while complying with HIPAA and agency policies.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 0-2 years of infusion nursing experience required.
• Current CPR certification required (an in-person class is required per company policy)

Responsibilities

• Initiates, develops, and implements nursing plan of care; evaluates patient progress.
• Assesses patient needs and status at each visit; revises plans per physician orders.
• Provides teaching for independence with prescribed therapy to patient and family.
• Communicates with Clinical team and documents care in real-time to support billing and continuity of care.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Adheres to HIPAA, agency policies, and professional standards in all activities.
• Performs other related duties as directed by supervisor.
