Job summary

Naven Health, Inc. is seeking an Infusion Nurse I to provide direct patient care in the home or alternate infusion sites under supervision. This licensed RN role focuses on safety and continuity of care under a physician’s plan.

Responsibilities include initiating care plans, educating patients and families, and coordinating with the clinical team. Travel up to 100% is required, with emphasis on accurate documentation and compliance with HIPAA and policy standards.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 0-2 years of infusion nursing experience.
• Current CPR certification required (in-person class per policy).

Responsibilities

• Initiates, develops, and implements basic nursing plan of care treatments and educates patients and families.
• Assesses patient needs and status at each visit and documents changes for physician review.
• Provides teaching to achieve independence with prescribed therapy and care needs.
• Communicates with clinical team, patients, and families to ensure continuity of care and timely documentation.
• Coordinates discharge planning and prepares discharge instructions with progress notes.
• Demonstrates compliance with policies, regulations, and quality standards.
• Maximizes work efficiency using computer systems to document care and schedule visits.
• Responds to patient requests and initiates emergency procedures as needed.
• Maintains professional conduct and HIPAA compliance.
• Informs patients about Naven Health services and plan of care.
• Attends training and mentoring activities as assigned.
• Participates in on-call responsibilities as applicable.
• Performs other related duties as directed.
