Job summary

Liberty Nursing Agency Inc is seeking a dedicated Home Infusion Nurse to deliver high-quality, patient-centered infusion therapy in patients’ homes. This role offers independent work and meaningful patient impact.

The ideal candidate will have an active New Jersey RN license, 1–2 years infusion experience, and strong clinical assessment and education skills. Travel to patient homes with a valid driver’s license is required.

Responsibilities

• Administer IV infusion therapies in patients' homes.
• Assess, monitor, and document patient responses to infusion treatments.
• Insert, maintain, and troubleshoot peripheral IV access devices (PICC lines/ports) as trained.
• Educate patients and caregivers on infusion therapy and home care protocols.
• Coordinate with physicians, pharmacists, and care teams for seamless patient care.
• Maintain accurate clinical documentation in compliance with standards.
• Manage infusion supplies for each patient visit.

Liberty Nursing Agency Inc is looking for a dedicated and compassionate Home Infusion Nurse to join our team. Responsibilities:

• Administer IV infusion therapies including antibiotics, hydration, IVIG and specialty medications in patients' homes
• Assess, monitor, and document patient responses to infusion treatments and report changes in condition
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)
• Educate patients and caregivers on infusion therapy, medication management, and home care protocols
• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care
• Maintain accurate clinical documentation in compliance with regulatory and agency standards
• Manage infusion supplies for each patient visit

Requirements:

• Active RN license in the state of New Jersey
• Minimum 1-2 years of infusion therapy, IV therapy, or home health nursing experience preferred
• Proficiency in peripheral IV insertion (Port and PICC experience a plus, will train)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for home visit travel
• Ability to work independently and manage a flexible patient schedule
