Job summary

Liberty Nursing Agency Inc. seeks a dedicated Home Infusion Nurse to deliver high‑quality infusion therapy in patients' homes.

You will work independently to administer IV therapies, assess responses, and coordinate with physicians and care teams to ensure exceptional patient care. The role requires an active NJ RN license with 1–2 years of infusion/home health experience or training in peripheral IVs and CVADs.

Qualifications

• Active RN license in NJ required.
• 1–2 years infusion/IV/home health nursing preferred.
• Proficiency in peripheral IV insertion; Port/PICC experience a plus (training provided).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver’s license for home visits.
• Ability to work independently and manage a flexible patient schedule.

Responsibilities

• Administer IV infusion therapies in patients' homes (antibiotics, hydration, IVIG, specialty meds).
• Assess, monitor and document patient responses to infusion treatments.
• Insert and maintain peripheral and central venous access devices.
• Educate patients and caregivers on infusion therapy and home care protocols.
• Coordinate with physicians, pharmacists, and care teams for seamless patient care.
• Maintain accurate clinical documentation per regulatory standards.
• Manage infusion supplies for each patient visit.

Responsibilities:

• Administer IV infusion therapies including antibiotics, hydration, IVIG and specialty medications in patients' homes


• Assess, monitor, and document patient responses to infusion treatments and report changes in condition


• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)


• Educate patients and caregivers on infusion therapy, medication management, and home care protocols


• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care


• Maintain accurate clinical documentation in compliance with regulatory and agency standards


• Manage infusion supplies for each patient visit

Requirements:

• Active RN license in the state of New Jersey


• Minimum 1-2 years of infusion therapy, IV therapy, or home health nursing experience preferred


• Proficiency in peripheral IV insertion (Port and PICC experience a plus, will train)
