Job summary

Liberty Nursing Agency Inc is seeking a dedicated Home Infusion Nurse to deliver high-quality, patient-centered infusion therapy in patients' homes across New Jersey. You will work independently, assess and document responses, and coordinate with physicians and care teams.

Requirements include an active New Jersey RN license, 1–2 years of infusion or home health nursing, and reliable transportation. This role offers flexible scheduling and the opportunity to make a meaningful impact in patients'

Qualifications

• Active RN license in New Jersey is required.
• Minimum 1–2 years of infusion therapy, IV therapy, or home health nursing experience preferred.
• Proficiency with peripheral IV insertion; Port/PICC experience is a plus and training is provided.

Responsibilities

• Administer IV infusion therapies in patients' homes, including antibiotics, hydration, IVIG and specialty meds.
• Assess, monitor, and document patient responses to infusion treatments.
• Insert and troubleshoot peripheral and central venous access devices.
• Educate patients and caregivers on infusion therapy and home care protocols.
• Coordinate with physicians, pharmacists, and care teams to ensure seamless care.
• Maintain clinical documentation compliant with regulatory standards.
• Manage infusion supplies for each patient visit.

Responsibilities:

• Administer IV infusion therapies including antibiotics, hydration, IVIG and specialty medications in patients' homes
• Assess, monitor, and document patient responses to infusion treatments and report changes in condition
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)
• Educate patients and caregivers on infusion therapy, medication management, and home care protocols
• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care
• Maintain accurate clinical documentation in compliance with regulatory and agency standards
• Manage infusion supplies for each patient visit

Requirements:

• Active RN license in the state of New Jersey
• Minimum 1-2 years of infusion therapy, IV therapy, or home health nursing experience preferred
• Proficiency in peripheral IV insertion (Port and PICC experience a plus, will train)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for home visit travel
• Ability to work independently and manage a flexible patient schedule
