Job summary

Liberty Nursing Agency Inc is seeking a dedicated Home Infusion Nurse to deliver high-quality, patient-centered infusion therapy in patients' homes. This role offers independent work in a flexible schedule, with opportunities to educate patients and caregivers and coordinate with the care team.

The ideal candidate is a skilled RN with experience in infusion therapy and strong clinical assessment abilities. Travel to patient homes is required with reliable transportation and a valid driver's

Qualifications

• Active RN license in New Jersey is required.
• 1–2 years infusion therapy, IV therapy, or home health nursing experience preferred.
• Proficiency in peripheral IV insertion; Port/PICC experience a plus; will train.

Responsibilities

• Administer IV infusion therapies in patients' homes (antibiotics, hydration, IVIG, specialty meds).
• Assess, monitor, and document patient responses; report changes in condition.
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines).
• Educate patients and caregivers on infusion therapy, medication management, and home care protocols.
• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care.
• Maintain accurate clinical documentation in compliance with regulatory and agency standards.
• Manage infusion supplies for each patient visit.

Requirements:

• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for home visit travel
• Ability to work independently and manage a flexible patient schedule
