Job summary

Liberty Nursing Agency Inc is seeking a dedicated Home Infusion Nurse to deliver high-quality infusion therapy in patients' homes. You will work independently to provide patient-centered care, administer IV therapies, monitor responses, and educate patients and caregivers.

The role requires NJ RN licensure, 1–2 years infusion/home health experience, and strong clinical assessment skills. Travel to patients’ homes with flexible scheduling and a supportive team environment is offered.

Qualifications

• Active RN license in New Jersey.
• 1–2 years infusion therapy or home health nursing preferred.
• Proficiency in peripheral IV insertion; Port/PICC experience a plus (training provided).
• Strong clinical assessment and critical thinking.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver’s license for home visits.

Responsibilities

• Administer IV infusion therapies in patients' homes (antibiotics, hydration, IVIG, specialty meds).
• Assess and document patient responses; report changes in condition.
• Insert and manage PICC lines, ports, central lines as needed.
• Educate patients and caregivers on infusion therapy and home care protocols.
• Coordinate with physicians, pharmacists and care teams for seamless care.
• Maintain compliant clinical documentation and regulatory standards.
• Manage infusion supplies for each patient visit.

Responsibilities:

• Administer IV infusion therapies including antibiotics, hydration, IVIG and specialty medications in patients' homes


• Assess, monitor, and document patient responses to infusion treatments and report changes in condition


• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)


• Educate patients and caregivers on infusion therapy, medication management, and home care protocols


• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care


• Maintain accurate clinical documentation in compliance with regulatory and agency standards


• Manage infusion supplies for each patient visit

Requirements:

• Active RN license in the state of New Jersey


• Minimum 1-2 years of infusion therapy, IV therapy, or home health nursing experience preferred


• Proficiency in peripheral IV insertion (Port and PICC experience a plus, will train)
