Liberty Nursing Agency Inc in New Jersey seeks a dedicated Home Infusion Nurse to deliver patient-centered infusion therapy in patients' homes. This role suits a nurse who thrives in independent settings and values meaningful, flexible work that makes a real difference.

You will administer IV therapies, monitor responses, manage venous access devices, educate patients, coordinate with physicians and care teams, and maintain accurate documentation while upholding regulatory standards.

Qualifications

• Active RN license in New Jersey.
• 1-2 years infusion/home health experience preferred.
• Proficiency in peripheral IV insertion; Port/PICC experience a plus (will train).
• Strong clinical assessment and critical thinking; excellent patient education.

Responsibilities

• Administer IV infusion therapies in patients' homes (antibiotics, hydration, IVIG, specialty meds).
• Assess, monitor, document patient responses; report changes.
• Insert, maintain, troubleshoot peripheral/central venous access devices.
• Educate patients and caregivers on infusion therapy and home care protocols.
• Coordinate with physicians, pharmacists, and care teams for seamless care.
• Maintain accurate documentation in line with regulatory standards.
• Manage infusion supplies for each visit.

Responsibilities:

• Administer IV infusion therapies including antibiotics, hydration, IVIG and specialty medications in patients' homes


• Assess, monitor, and document patient responses to infusion treatments and report changes in condition


• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)


• Educate patients and caregivers on infusion therapy, medication management, and home care protocols


• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care


• Maintain accurate clinical documentation in compliance with regulatory and agency standards


• Manage infusion supplies for each patient visit

Requirements:

• Active RN license in the state of New Jersey


• Minimum 1-2 years of infusion therapy, IV therapy, or home health nursing experience preferred


• Proficiency in peripheral IV insertion (Port and PICC experience a plus, will train)


• Strong clinical assessment and critical thinking skills


• Excellent communication and patient education abilities


• Reliable transportation and valid driver's license for home visit travel


• Ability to work independently and manage a flexible patient schedule
