Job summary

Liberty Nursing Agency Inc is seeking a dedicated Home Infusion Nurse to deliver high-quality infusion therapy in patients' homes. This role offers independence, flexible scheduling, and meaningful patient care.

Ideal candidates have an active NJ RN license and 1–2 years of infusion/home health experience, with training available for Port/PICC insertion. Travel is required to visit patients in their homes.

Qualifications

• Active RN license in New Jersey.
• Minimum 1–2 years infusion therapy or home health nursing preferred.
• Proficiency in peripheral IV insertion; Port/PICC experience a plus (will train).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Reliable transportation and valid driver's license for home visits.
• Ability to work independently and manage a flexible patient schedule.

Responsibilities

• Administer IV infusion therapies including antibiotics, hydration, IVIG and specialty medications in patients' homes.
• Assess, monitor, and document patient responses to infusion treatments and report changes in condition.
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines).
• Educate patients and caregivers on infusion therapy, medication management, and home care protocols.
• Coordinate with physicians, pharmacists, and care teams to ensure seamless patient care.
• Maintain accurate clinical documentation in compliance with regulatory and agency standards.
• Manage infusion supplies for each patient visit.
