Job summary

Ennoble Care is hiring a full‑time Hospice RN Case Manager who resides within or near Easton, PA, to join the interdisciplinary team delivering skilled nursing assessments, planning, and care to maximize patient comfort and health for patients and their families. The role reports to the Director of Nursing and collaborates with the attending physician and hospice staff to coordinate comprehensive care.

Qualifications

• Valid PA RN license.
• 1–2+ years nursing, hospice experience preferred.
• Ability to assess patient and caregiver needs.
• Driver’s license and reliable vehicle.
• Team-oriented with interdisciplinary care approach.

Responsibilities

• Manage assigned patient caseload across all phases of care.
• Document notes in EMR accurately and timely.
• Coordinate care with physicians, hospice staff, and agencies.
• Educate families on levels of care and support.
• Attend patient care conferences and support leadership feedback.
• Supervise NAC every two weeks or as assigned.
• Provide off-hours coverage as requested.
• Support family bereavement and time of death as needed.
