Job summary

Ennoble Care is seeking a Full-time Hospice Registered Nurse Case Manager to serve the Arlington County region in Virginia. The RN Case Manager will assess, plan, implement, and evaluate patient care as part of the interdisciplinary team under the Director of Nursing.

You will coordinate with physicians, Hospice staff, and other agencies; maintain EMR documentation; and support patients and families through bereavement and care planning. A VA RN license and driving ability are required.

Qualifications

• Possession of a current Licensed Registered Nurse (RN) license in Virginia.
• At least two years of professional nursing experience, hospice experience preferred.
• Ability to assess and respond to patients and caregivers' needs in varied settings.
• Skilled nursing practice with ability to document clear, accurate reports.
• Must be a team player supporting an interdisciplinary care approach.
• Must have a valid driver’s license, reliable vehicle, and year‑round driving ability.
• Ability to work independently and within a multidisciplinary team.

Responsibilities

• Manage a patient caseload with the nursing process phases (assess, plan, implement, evaluate).
• Coordinate with attending physicians, Hospice staff, and other agencies to optimize care.
• Maintain regular communication with the attending physician and care team.
• Document accurately in electronic medical records and participate in interdisciplinary meetings.
• Provide patient/family education and support, including bereavement and end‑of‑life care discussions.
• Attend patient care conferences and contribute to team discussions and problem‑solving.
