Job summary

HealthFlex Home Health & Hospice is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to provide skilled nursing care and coordinate patient plans of care. The role supports patients and families through the End of Life process with a focus on compassionate, individualized care.

Schedule options include Full Time, Part Time or Per Diem, with opportunities for flexible scheduling and remote work as appropriate.

Qualifications

• Current CA license as registered nurse.
• 1+ year hospice case management experience.
• Current AHA CPR certification.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provides professional nursing care using nursing process and plans of care.
• Initiates and revises the plan of care with patient and family involvement.
• Re-evaluates patient and family/caregiver needs and updates care plans as necessary.
• Administers medications and treatments per physician orders and plans of care.
