Job summary

St. Mary's Home Care and Hospice in Bucks County, PA is seeking a full-time Hospice RN Case Manager to provide professional nursing care to hospice clients as prescribed by the physician and in line with our policies.

The RN will conduct comprehensive assessments, develop and implement care plans, coordinate with an interdisciplinary team, and oversee patient outcomes. Collaboration with physicians and Family/Caregivers is essential to deliver compassionate, home-based care.

Qualifications

• Graduate of an approved nursing education program.
• Licensed as a Registered Nurse in Pennsylvania.
• 2 years experience as a professional nurse in acute care, home care or ER.

Responsibilities

• Assess and plan patient care; implement interventions and supervise outcomes.
• Serve as case manager for hospice patients and coordinate with physicians.
• Participate in weekly Interdisciplinary team meetings.
