Job summary

St. Mary’s Home Care and Hospice in Bucks County, PA is seeking a full-time Hospice RN Case Manager to provide nursing care to hospice clients as prescribed by physicians and in alignment with state regulations.

You will assess, plan, intervene, and supervise patient care and outcomes and collaborate with an interdisciplinary team. The RN will participate in weekly Interdisciplinary meetings and coordinate with the treating physician or hospice medical director to manage patient symptoms.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the State of Pennsylvania.
• 2 years experience as a professional nurse in acute care, home care or ER.
• Home care experience preferred but not required.
• EPIC experience preferred.

Responsibilities

• Provide professional nursing care to hospice clients as prescribed by the physician and in accordance with policies.
• Assess, plan, implement, and supervise patient care and outcomes in collaboration with the interdisciplinary team.
• Attend weekly interdisciplinary meetings and coordinate with primary care physician or hospice medical director.
