Job summary

Ennoble Care is seeking a Hospice Registered Nurse Case Manager to cover the Camden County, NJ region. This full-time role involves assessing, planning, and coordinating care for patients at home, in collaboration with the interdisciplinary team and attending physicians.

The ideal candidate is a NJ-licensed RN with at least 1 year of experience (hospice preferred), strong assessment skills, and fluency in English and Spanish. On-call responsibilities and travel to patient homes are required.

Qualifications

• Graduate from an accredited School of Nursing.
• Licensed RN in New Jersey (NJ) per NJ Board of Nursing.
• One year of RN experience; hospice experience preferred.
• Excellent assessment and problem-solving skills.
• Strong organizational and time-management abilities; knowledge of case management processes.
• Must have a valid driver’s license with an insured vehicle.
• Fluent in English and Spanish.

Responsibilities

• Manage a patient caseload including assessment, planning, implementation, and evaluation.
• Coordinate care by communicating with attending physicians, Hospice staff, and other agencies.
• Document accurately in electronic medical records.
• Participate in Interdisciplinary Group meetings and care conferences.
• Provide caregiver education and Bereavement support.
• Respond to on-call needs and assist during patient/family issues.
