Job summary

Ennoble Care is seeking a Hospice Registered Nurse Case Manager to serve patients in Middlesex / Somerset County, NJ, delivering in-home assessments and coordinating care with an interdisciplinary team. The role involves managing a caseload, communicating with physicians and hospice staff, and ensuring high-quality, compassionate end-of-life care.

The ideal candidate is a NJ-licensed RN with at least one year of nursing experience, hospice preferred, and strong organizational and communication

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Licensed in NJ as a Registered Nurse by the New Jersey State Board of Nursing.
• One year of RN experience; Hospice experience preferred.
• Excellent assessment and problem-solving abilities.
• Strong organization, time management, and case management knowledge.
• Must have reliable transportation and a valid driver's license.
• Strong verbal and written communication skills.

Responsibilities

• Provide skilled nursing assessments, planning, and care to maximize patient comfort and health.
• Identify patient and caregiver needs and deliver supportive care per physician orders and plan of care.
• Manage a patient caseload across assessment, planning, implementation, and evaluation phases.
• Coordinate with attending physicians, hospice staff, and other agencies for optimal care.
• Maintain timely documentation in electronic medical records.
• Participate in Interdisciplinary Group meetings and care conferences.
• Educate caregivers and, when needed, provide direct care.
• Assist in on-call coverage and respond to patient/family needs during absence of other nurses.
• Support at time of death and bereavement period.
• Engage in orientation, training programs, and community outreach as required.
