Job summary

Ennoble Care is hiring for a full-time Hospice Registered Nurse Case Manager in the Camden County, NJ region. The RN Case Manager works within the interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize comfort for patients and families.

Responsibilities include managing patient caseload, coordinating with physicians and Hospice staff, documenting in the EMR, and participating in Interdisciplinary Group meetings.

Qualifications

• Must be a graduate from an accredited School of Nursing.
• Must hold NJ RN license and be in good standing.
• One year hospice or similar experience preferred; bilingual ability is a plus.

Responsibilities

• Manage an assigned patient caseload and coordinate care across the care team.
• Communicate with attending physicians, Hospice staff, and related agencies.
• Document accurately in the electronic medical record (EMR).
• Participate in Interdisciplinary Group meetings and care conferences.
• Provide support to patients and families during bereavement and care transitions.
