Job summary

CenterWell Home Health is seeking a licensed Speech Language Pathologist to deliver in‑home speech pathology services, participate in treatment planning, and coordinate care with a care team. You will assess and treat communication and swallowing disorders, maintain documentation, and support patients and families in achieving functional goals.

The role requires ASHA CCC‑SLP/CCC‑A credentials, a current license, CPR, and reliable transportation for home visits in New Jersey.

Qualifications

• Must meet education and experience requirements for CCC‑SLP/CCC‑A with ASHA
• Minimum six months experience as a speech language pathologist
• Current and unrestricted licensure to practice
• Current CPR certification
• A valid driver’s license and reliable transportation required

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in home or facility
• Participate in development and periodic review of the Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing
• Administer and interpret diagnostic tests and applications of therapeutic treatments
• Observe, record and report changes in patient condition to supervisor/physician
• Provide instruction in alternative communication methods when appropriate
• Counsel patients, families and healthcare staff
• Maintain and submit documentation and progress reports as required
• Participate in care coordination and discharge planning
